List of Your Current Medications

You will need to provide all medications and vitamins you are currently taking. Keep in mind medications must be tried for 30 days before ordering.

Photo ID

An image of your valid government-issued photo ID is required for registration. This includes:

Driver’s License (Issued within U.S. including U.S. Territories)
Non-Driver Photo ID (Issued within U.S. including U.S. Territories)
U.S. Passport
U.S. Passport Card
U.S. Military ID Card
U.S. Military Dependent’s ID Card
Tribal Card

Prescription For a 3-Month Supply With 3 Refills

Ask your prescriber for a prescription for a 3-month supply with 3 refills. If you don’t have it, we will reach out to the prescriber on your behalf.

Submit Prescription by Fax or Mail

We accept original prescriptions by mail from the prescriber or from the member. We only accept faxes directly from the prescriber’s office.
